CORI Process for Wakefield Soccer Association

Introduction

Greetings, and thank you for volunteering for the Wakefield Soccer Association.

I am the CORI submitter for Wakefield soccer and am in charge of making sure everyone who has any contact with children does a CORI check. Please follow the steps below and we will be able to fulfill the requirements of the state law and of Mass. Youth Soccer.

Process

Please follow these three steps to complete the CORI process.

1) Enter or update your personal information in the Sportsmanager system including your name, email address, date of birth, gender, and phone numbers. Use this Parent's Guide to SportsManager to get into the system. Use the Edit Adult Info button next to your name to enter/update your data.

2) Go to the state soccer website to complete and submit the form. When asked for "name of organization", enter "Wakefield Youth Soccer".

3 ) A copy of your driver's license is needed only one time. If you have previously had your license scanned by WSA, you do not need to do it again. Future CORI renewals do not require another copy unless information on your license has changed since the time when you last submitted it.

If you need to submit a copy of your license, you may use one of these options:

A) Contact Michael Kilkelly, riskmgmt@wakefieldsoccer.org , and arrange to meet at a WSA monthly general meeting (usually held the first Thursday of each month). Bring your license with you.

B) Email a scanned copy to riskmgmt@wakefieldsoccer.org

C) Drop off or mail a copy to: Michael Kilkelly, 37 Lawrence Street, Wakefield.
